As soon as you have placed the order for your Discovery Session, we will email you 5 documents. 

Four of these documents need to be completed for our first session. 
You may bring them along if we are meeting in person, or email/fax them to us if we are meeting via phone.

The fifth document, the Wheel of Life, is something we will work with during our first session. 

Step 3
Complete your Discovery Session. This session will be 60 to 120 minutes and will give us both a chance to evaluate where you are now and where you want to be in life. 

Please come prepared for this session. Choose any topic you'd like to clarify or improve in your life.

Step 4
The journey begins NOW!! 

Decide to improve and change your life. Make a decision that will alter your future forever! 

Proceed with Coaching and see how far you can go, how much you can improve, how exciting your life can become. 

...Or say "no thank you" and go back to what you were already doing - knowing that you will have walked away from a potentially life altering experience.

If you have made it to Step 5, you have my hearty CONGRATULATIONS on making a decision to do something for yourself.

Initial Intake Information & Questionnaire is Required 

I begin the process of working on your file as soon as you complete and submit this simple form. I then analyze the information to get a better feeling for who you are, what makes you tick and what you believe. I then compile a permanent record in my system, so I always have backup and readily available information when you contact me. I want to be sure and be as thorough as possible and this intake form has proven invaluable in helping me 'get to know' everyone I work with. 

This is a short exercise that will take you 15 to 30 minutes to complete

There is a nominal $19.99 fee to submit this information.

Why is there a fee charged, just to submit a form?

My time is extremely valuable to me and I make my living charging for this time. This form will take at least an hour of my time to analyze and scrutinize to determine your personal traits, characteristics, wants, needs, learning style and personal values. These are all key to helping me know the best way to work with you and understanding you better.

If everyone was able to submit this form without cost, I’d spend all day, every day, sifting through countless forms submitted by 'tire kickers' and I'd 
never have time to coach and earn a living. Despite how much I love what I do, I still have to pay my bills  ; >) By charging a minimal fee we eliminate most of those who aren’t serious. This allows us to better help you and not waste time on those who aren't serious. The fee is nominal and doesn't nearly cover the time I will actually spend on this task.
